Peer Connections and Living Well in North Carolina
What is Peer Connections?
We will work with Real Advocates now Emerging (RANE) to help self-advocates live healthier, safer lives by increasing access to information, resources and social support. The project will last 12 months, starting in October 2006. It is funded by the North Carolina Council on Developmental Disabilities. Our organization is the Self-Advocacy Leadership Network (SALN). The project will be managed by our staff.
What will participants do in the Peer Connections project?
We will focus our work in Carrboro and Chapel Hill. In this area we will work with 30 self-advocates interested in participating in the project. These individuals will be divided into three groups of 10 each.
Each group will meet once every month to:
Learn about healthy lifestyles;
Discuss and identify their wellness goals and wellness activities of interest;
Identify resources that can help them achieve their wellness goals;
Review their progress periodically, and Identify barriers to accessing wellness and other recreational activities in the community
